Entrymark documentation

Entrymark adds two macros to Confluence Cloud: Entrymark form (build a form on a page and collect responses) and Entrymark responses report (show a form's responses on any page). Responses are stored by Entrymark on your Confluence site (Forge storage), never in the page body.

Add a form

  1. Edit a page or blog post, type /Entrymark and choose Entrymark form.
  2. Publish the page. Forms are built and filled on the published page.
  3. On the page, click a template (Event sign-up, Feedback, Request intake) or Blank form. The person who builds the form becomes its owner.

Build and change the form

Click Edit form (page editors and the owner). The builder has three tabs.

Questions. Pick a type and click Add question. Types:

Move questions with Up and Down. A question's type can't change once it exists (stored responses depend on it); add a new question instead. Deleting a question that already has answers keeps those answers in the CSV export as "(deleted question)".

Show-when rules. Open a question and click Add rule to show it only when an earlier answer matches: is, is not, contains, does not contain, is checked, is not checked, is answered, is not answered, more than / after, less than / before. Up to five rules, with All rules match or Any rule matches. Rules can only look at questions above, and a hidden question counts as unanswered for the rules below it. Answers to hidden questions are never stored. Save once before using a new question in a rule.

Settings. Title, intro text, one response per person or many, whether people can edit or withdraw their own responses while the form is open, close date (end of that day, UTC), response limit, and the message shown after sending.

Privacy and notifications (the owner only).

  • Who can see responses: Only the form owner, The owner and people who can edit this page, or Everyone who can view this page. People always see their own named responses.
  • Anonymous: no account ID is stored with responses and only the day is kept. It locks once the first response arrives. Anonymous forms don't allow editing responses, because nobody knows which response is whose. On one-response-per-person anonymous forms Entrymark keeps a separate one-way mark per person, so deleting an anonymous response doesn't let that person respond again.
  • Notify about new responses: hourly digest, daily digest or off. Entrymark starts one comment thread on the page and replies to it, @mentioning the owner and up to 10 more people, so Confluence notifies them. Digests show counts, never answers.
  • Form owner: hand the form to someone else. On "Only the form owner" forms, other page editors can't change the form at all.

If two people edit the form at once, the second save is refused with a note to reload, so nobody's changes are silently overwritten.

Fill in a form

Anyone who can view the page and is logged in can respond while the form is open. Required questions are marked with *. Mistakes are shown next to each question. After sending, the page shows Your response with Edit and Withdraw when the form allows it. Forms that take many responses show Send another response.

Work with responses

Click Responses on the form (or add a report, below).

  • Filter by status (default: everything but Archived), text search across answers and names, submitted date range, and any question (is, contains, at least, at most).
  • Sort by submitted time, status, respondent or any question. Unanswered values sort last.
  • View opens a response with every answer.
  • Statuses: New, Seen, Done, Archived. Select rows and mark them in one go. An edited response comes back as New.
  • Summary: answers per question; total, average, lowest and highest for numbers and ratings; counts per option.
  • Charts: bar or pie per choice, checkbox and rating question, and responses per day.
  • Export CSV: every response that matches the current filters and sort. Copy it from the dialog into Excel or Google Sheets, or save it as a .csv file.
  • Delete selected responses (people who manage the form).

Manage

Manage on the form shows the owner, the report code, Close now / Reopen, Follow the close date again, and (owner) Delete form and responses. A full form stays closed until you raise its limit. If nobody owns a form (for example a copied page), any page editor can click Become the owner.

Responses report macro

Insert Entrymark responses report on the same page as a form, or on any other page with the form's report code (Manage, Report code) in the macro settings. Choose whether it opens on Table, Summary or Charts. The report follows the form's privacy: people who can't see the responses on the form can't see them in the report either, and they also need to be able to view the form's page.

Copies and templates

Copying a page copies the form's questions and settings (never responses, owner or notify list) when the copy is first opened, provided the person opening it can see the original page. If they can't, the copy starts as an empty form. A page editor then clicks Become the owner.

Licensing

Entrymark is paid via Atlassian with a 30-day free trial. If the license lapses, forms and responses stay readable and exportable; new responses, edits and form changes pause until the license is renewed.
